Most Path patients can use the Path patient portal to update their address and phone number on file without needing to contact Customer Support! Please follow the below instructions to update this information.
At this time, patients cannot update their legal name nor email address on file using the Path patient portal. If you need to update your name or email address, please email us at support@pathccm.com and one of our team members will be happy to assist you.
Updating Your Address and Phone Number

Log into the Path patient portal here. If you need help doing this, please follow these instructions.
If you are unable to access the Path patient portal after following the instructions, please email us at support@pathccm.com and one of our team members will be happy to assist you.
Once logged in, complete the steps below to update your insurance information on file with Path.
- Click Profile on the lefthand side of the screen to show your account details.
- Click Edit in the Personal details section.
- Enter any of the following:
- Phone number
- Street address/Apt #
- City
- State
- Zip code
- Click the purple Save button to save your information.
Your personal details are now updated in Path’s system and will be used going forward.
