Logging Into the Path Patient Portal

Great news - most Path patients can use the Path patient portal to self-serve various tasks without contacting Customer Support! Click here to learn more about what tasks can be completed in the Path patient portal. 

The instructions below show how to log into the Path patient portal.

If you cannot access the Path patient portal after following the instructions below, please email support@pathccm.com, and one of our team members will be happy to assist you.

Logging into the Path patient portal

Patients can access the Path patient portal by following this link. Even if you have not completed your first visit with a Path therapist, you can still access the Path patient portal!

Alternatively, patients can access the Path patient portal from our website at www.pathmentalhealth.com. Even if you have not completed your first visit with a Path therapist, you can still access the Path patient portal!

At www.pathmentalhealth.com, select the Patient login button in the upper right-hand corner of the screen - this will take you to the Path patient portal login page.

You'll then enter the email address on file with Path in the "Sign in" screen.

Once you enter your email address and submit the "Continue with email" button, you'll receive an email with a unique link to access the Path patient portal. The email will be from Path Mental Health, and the personalized link will bring you to your Path patient portal! 

Note: No password is required to access the Path patient portal, and the login link sent via email expires after five minutes.

Success! Now that you are logged in, you can perform any of the actions below. Click any link from the below list to access instructions on completing each step.
